but dont we use E-Z conf if there are 4 different groups attached to a doubly bonded C??
or we can use it in such cases also?

yeah we can.....
just we have to see that the isomers cannot be superimposed by any means
but yeah more than 2 grps has to be present else we have cis-trans only!!
3 grps give cis-trans as well as E-Z as in this question
for 4 grps we have only E-Z!!
