During fever the thermostat in the hypothalamus is set to a new level i.e. a higher level. The body then acts like it does at clod environmental temperatures. It generates heat by increasing metabolic rate, increases shivering and also does vasoconstriction. Vasoconstriction reduces blood flow to the extremities (hands and feet), and hence these get cold...just as in cold weather.
